Fear of seeking help

Ask how a service handles your information before you share anything personal.

Many pharmacists, doctors and nurses delay getting help for stress, depression or alcohol use because of one fear: the regulator or the employer will find out. Some worry about a record on a shared clinical system. Others work in the same building as their own GP. The fear is understandable. The facts are often less frightening than the worry, and several services exist for this exact problem.

Short answer

A health condition on its own is not a fitness to practise concern. The GMC needs to know about a doctor's health only if the condition poses a risk to patients. Confidential services such as NHS Practitioner Health in England, Canopi in Wales and the Workforce Specialist Service in Scotland exist for staff who cannot get care confidentially through local services.

The common fears, answered

"My regulator will find out."

Regulators do not receive a list of people who see a GP or a counsellor. The GMC's information for doctors considering self-referral says a doctor does not need to self-refer when there are no conduct concerns and no risk to clinical care. A referral matters where a condition is recent, poorly controlled or affecting work without proper management. Pharmacy and nursing professionals should read their own regulator's guidance on health, but the principle is the same: managing a condition responsibly is what regulators expect.

"My employer will be told."

Confidential services do not report to your employer as routine. Canopi is independent of the NHS and does not add to NHS medical records. Occupational health reports to your employer about fitness for work and adjustments, not your full history, Ask what a report will include and ask to read the report before release.

"My GP works with me."

GMC Good medical practice (paragraph 77) tells doctors to register with a GP outside their family and workplace. If your GP is a colleague or works in the practice you supply, register elsewhere or use a specialist service. NHS Practitioner Health and the Workforce Specialist Service exist for staff in this position.

"Asking for help means I'm not coping."

Every regulator expects you to look after your health. The GMC asks doctors to seek independent professional advice on fitness for work rather than relying on self-assessment (paragraph 78). The NMC Code asks nurses and midwives to maintain the level of health needed for their role (standard 20.9). Getting help is part of meeting the standard.

Questions to ask a service before you share details

  • Who sees my information, and is any of the record shared with my GP or employer?
  • In what circumstances would you break confidentiality? Most services name serious risk to you, to others or to patients.
  • Do you write to my regulator or employer, and would you tell me first?
  • Is the service free for my profession and location?
  • How long until a first appointment?

The smallest first step

You do not have to tell your whole story on the first call. Many people start with a confidential conversation about how the service works. Pharmacists, trainees and pharmacy students can call Pharmacist Support, and technicians should ask the charity whether they qualify. Doctors can call the BMA counselling line, open day and night. NHS staff in England who cannot get confidential care locally can refer themselves to NHS Practitioner Health. Community pharmacy staff should check eligibility with the service first. NHS staff in Wales can contact Canopi. NHS Practitioner Health treats addiction as well as mental illness.

Write the fear down

Write the outcome you fear most in one sentence. Then write the question which would settle the fear, such as "Will my employer see this?" Ask the service the question on the first call.

Do I have to tell the GMC about depression or anxiety?

Not if the condition poses no risk to patients. The GMC says doctors need to self-refer only where a health condition creates a risk to patients or has contributed to conduct or serious clinical concerns.

Is NHS Practitioner Health confidential?

Yes. NHS Practitioner Health is a confidential service for NHS staff in England who cannot get confidential care through local services. Ask the service directly about the limits of confidentiality before you share details.

Will seeing a counsellor affect my registration?

Seeing a counsellor or GP does not trigger a report to your regulator. Your regulator's interest starts only where a health problem puts patients at risk and is not being managed.

Where can a pharmacist get confidential help?

Pharmacist Support offers wellbeing help for pharmacists, trainees and students. Pharmacy technicians should ask the charity directly whether they qualify.
